Definition
The countercurrent mechanism generates and maintains an increasing osmolality gradient along the medullary pyramid, outer layer to innermost part, concentrating urine.
It depends on integrated functioning of the loop of Henle, vasa recta and collecting duct; medullary osmolality reaches about 1200 to 1400 mosm/kg of H2O innermost.
